COORDINATED SCHOOL HEALTH

* If you have a child that takes medication at school the deadline to pick up all meds is May 21, 2015.

* If you have a child that is currently in the 6th grade, they will need a Tdap booster, 2nd Varicella shot or date they had chicken pox, and a new TN. immunization record to enter the 7th grade. The form and proof of immunizations can be brought to the school during registration.

* If you have a child that will require care from a nurse during the 2015/2016 school year, please contact the Office of Coordinated School Health at 221-0959.

*If you have any concerns regarding the health and well being of your child, while in school, please contact Michelle Brazier RN, Director of Coordinated School Health, at 221-0959.




Coordinated School Health (CSH) provides that the overall school environment is coordinated within and between the school, home, and community to address health-related concerns and accomplish shared goals.
